The hearts@school Community Toolkit is a how-to guide designed to assist you with organizing a hearts@school event in your community. The toolkit consists of a manual and an appendix of resources.

The hearts@work Toolkit is an online catalogue of resources including:
- Event planning information for hearts@work coordinators
- Station guides and checklists for the nine hearts@work stations
- Reference materials and guidelines for health professionals
- Resource materials for hearts@work participants

The Heart Manual for Coronary Artery Disease (The 'Generic' Heart Manual) is a rehabilitation program appropriate for all cardiac patients.
The 'Generic' Heart Manual is:
- Home-based
- Nurse-facilitated
- Evidence-based
- A cognitive-behavioural self-management program
The 'Generic' Heart Manual is suitable for people who have had:
- Myocardial infarction / acute coronary syndrome / percutaneous coronary interventions (PCIs)
- Angina (stable)
- Heart failure (NYHA classes 1 and 2)
- Implantable cardiac defribillator (ICD)
- Coronary artery bypass graft (CABG)
- Angioplasty/stent
After completion of The Heart Manual Facilitator Course, Health Professionals become certified facilitators of The Heart Manual Program.

This program a home-based self-management rehabilitation program for patients recovering from a heart attack. Facilitators must be trained to use this manual with patients.
